Catatan pelajaran

Smallpox and vaccination

  • Jenner’s 1796 cowpox experiment and 1798 publication helped establish vaccination against smallpox. He was not the first person to use cowpox, and variolation using smallpox material was an earlier, distinct practice. Historical experimentation should not be read as a modern model of consent or clinical trial design.
  • Free provision developed under 1840 legislation; the 1853 Act imposed infant smallpox vaccination duties in England and Wales. Opposition had varied motives and enforcement changed over time. Neither policy dates nor historical safety disputes apply automatically to every modern vaccine.

Germ theory

  • Pasteur’s work on fermentation and spontaneous generation showed the importance of microorganisms in contamination. The development of disease explanations extended beyond one experiment or date; not all illness is infectious and not all microorganisms are harmful.
  • Koch connected specific bacteria with diseases, including anthrax in 1876, and developed laboratory methods. Rivalry, teams, funding and communication all contributed to progress; do not make competition the sole cause.

Surgery

  • Surgery faced pain, infection and bleeding. Ether, nitrous oxide and chloroform were used as anaesthetics; Simpson promoted chloroform in 1847, with serious risks. Lister’s carbolic antisepsis began in 1865 and was published in 1867. Later aseptic practice aimed to prevent contamination. These developments did not solve every surgical problem immediately.

Nursing and public health

  • Nightingale and her nurses arrived at Scutari in 1854; her training school opened in 1860. Her reforms concerned nursing, conditions and organisation. Mary Seacole provided care, food and support through a different independent role near the Crimean front. Their contributions should not be collapsed into one identical job.
  • Snow investigated the Broad Street cholera outbreak in 1854 and argued for waterborne transmission; the epidemic was already declining before the pump handle’s removal. His evidence was wider than that intervention. Chadwick’s 1842 report promoted sanitation within a different explanatory framework.
  • The 1848 Act created a limited central framework with local implementation and specified intervention powers; purely voluntary is an oversimplification. The 1875 Act consolidated stronger local sanitary duties in England and Wales.
  • The Great Stink of 1858 intensified political pressure around London’s sewers. Bazalgette’s interceptor scheme depended on planning, finance and years of building; 1858 is not the date that the entire system was completed.
